La cappella è situata in campagna, in una zona denominata "Campagna Colombo" o "Freifeld", termine tedesco, che indicava il "campo franco", il luogo cioè, dove una volta si svolgevano i duelli.
L'edificio sacro fu eretto, insieme ad una vicina villa, dall'olandese Giovanni Colombo, che per esaudire il desiderio della moglie, nativa di Gradisca, acquistò gran parte delle terre che oggi vengono appunto identificate come "Campagna Colombo".
La cappella fu dedicata alla madre della Madonna e nel corso del tempo ha accolto molti fedeli, specialmente nella ricorrenza di Sant'Anna, il 26 luglio.
Nel 1797, i Francesi, mentre avanzavano verso la fortezza, saccheggiarono la villa e profanarono la cappella, che fu successivamente riconsacrata.
Durante il primo conflitto mondiale la villa vicina fu distrutta e al suo posto fu costruito un altro edificio, tuttora esistente.
La cappella, che prospetta su un minuscolo cortile, è caratterizzata da un campaniletto, che per ben due volte è stato derubato e privato della campanella, che chiamava a raccolta i devoti.
